Memorial Service, 10am, Friday, July 17, at Shinkle Mortuary, 146 N. Lamar Ave, Haysville, KS.
Jack was preceded in death by his wife, Marsha Slade; mother, Ruby Slade. He is survived by his, daughter, Terena (Timothy) Barkley of Haysville; grandchildren, Gina, Emalee, Isaiah.
